A toe ring for the big toe? You may want to try taking her into a shop that fits you for a toe ring so that you can find one that is comfortable for her. Sometimes they have booths for custom fit toe rings at farmer's markets too.

If you're interested: Together with my girlfriend I have a jewelry workshop and one of the things I make are toe rings. I could sell you one of my own design or create one according to your wife's wishes.
